Previously, you were provided with Your Book of Benefits, a three-ring binder outlining the benefits available to you as an active employee. As legal and plan changes have occurred you have received replacement pages with updated information. It is once again time to update Your Book of Benefits.

Due to the many legal and plan design changes, it is necessary to replace ALL sections of Your Book of Benefits with current plan information.

To update Your Book of Benefits, simply remove the existing information behind each section tab and replace it with the enclosed information.

The Prescription Drug and Vision Care plan sections have been incorporated into the Medical Plan section. You will find a reminder page in each respective section indicating this change.

Please update Your Book of Benefits as soon as you receive this packet. It is important to have up-to-date information available for reference as needed.
